A 12-year-old Arizona boy left his middle school last week and has been reported missing.

Matthew Dubose left Arizona Desert Elementary School on Van Buren Street in Tolleson, Arizona (not far from Phoenix) on a black and red bicycle on May 11 at around 3:15 PM.

The boy is about 5 feet 7 inches, and has brown eyes and curly brown hair. He wears glasses. He was last seen wearing a school uniform – a blue polo shirt and tan-colored shorts, black Nikes – and was carrying a black backpack.

Police say they have a video of Matthew playing basketball after leaving the school.

“We are deeply concerned for Matthew’s safety and are doing everything we can to assist in the search,” Tolleson Elementary School District Superintendent Dr. Lupita Ley Hightower said in a statement Saturday.

“This is my living nightmare,” the boy’s mother, Monique Triplett DuBose, told Fox 10 Phoenix. “You don’t expect yourself to be in this position. Any mother who deals with this knows exactly what’s going through my head There aren’t even words to describe it, they feel the heartbreak.”

Matthew’s mother revealed that her son is diagnosed with ADHD, and has left home in the past, but never for this period of time.

Police are searching for the boy, who has not been heard from since leaving the school. Postings are all over social media and the internet. The public is asked to help by calling 623-936-7186 with any information or leads.
