O’Donnell underwent a lower deep-plane facelift in January 2026, a procedure that lifts tissue under the facial muscles to avoid the skin looking too tight. Rather than transform her entire face, she focused only on rejuvenating her lower face and neck.

“Authenticity is the goal in these days and times,” she said. “And I think all that matters is truth and love, and so I wanted to be truthful and say all the complicated emotions I had about it.”

O’Donnell, who has Type 2 diabetes, began taking the GLP-1 medication Mounjaro in late 2022 and has lost more than 50 pounds over the past three years. In Ireland, people would repeatedly ask if something was wrong because the weight loss left extra skin around her face and created lines that gave her a perpetually sad expression. “In Ireland, people would say, ‘Are you upset, darling? What’s the matter, love?’ And I’m like, ‘That’s just my face. I am not upset; it’s just how I look,’” she shared.

In her Substack essay, O’Donnell described her face as having taken on a “haunted” look after the weight loss, and admitted her face had begun “melting.”

An Honest Conversation With Clay

O’Donnell also opened up about the conversations she had with her 13-year-old child, Clay, before going through with the procedure. “My little autistic 13-year-old said to me, ‘If you do it, I would not be able to respect you ever again,’” she recalled. “And that put me off it for a few months, but then when I went and did it in January, I came home 10 days later, and they never noticed.”

O’Donnell said her goal was to model bodily autonomy for her child, explaining that she didn’t want Clay to learn that her body belonged to any ideology—even a well-intentioned one like feminism. She added that the procedure left her looking like a slightly more rested, emotionally steadier version of herself.

In her essay reflecting on the decision, the comedian acknowledged she had once been firmly against facelifts. “I used to feel very strongly about facelifts. Not casually — morally. I had assigned myself as head of all women who would never — ever,” she wrote. “I thought it was a betrayal. Of feminism. Of aging. Of our team of women worldwide. And then I lost 50 pounds…”
